Fingerprinting Requirements: ^{must meet ALL requirements}

You must provide current government-issued photo identification, such as your driver’s license.  The information submitted on this form and on the fingerprint card must match your identification.

Pricing:

Fingerprinting costs $20.00 for up to two (2) fingerprint cards.  Additional cards are $5.00 each.  Appointments for fingerprinting will be processed on Tuesdays through Fridays between 9:00 AM and 10:00 AM.  Please note you will be asked to pay now to reserve your appointment time.
